Applications and Requirements

Osteopathic and Allopathic students/applicants are to apply to the La Grange Memorial Hospital Family Medicine Residency Program through ERAS (Electronic Residency Application Service) via the U.S. Medical Schools and the ECFMG. PAPER APPLICATIONS WILL NOT BE ACCEPTED

Requirements for U.S. and IMG Applications

  • Graduated from your medical school within two years of application.
  • If you have been out of medical school longer than two years, we look at applicants who have been in the active practice of medicine or residency in the past two (2) years in an ACGME or OGME approved program.
  • If you have not been enrolled in a formal program of medical education or cannot demonstrate active practice of medicine within the two years immediately preceding application for Illinois licensure, you must demonstrate professional capacity. Please see The Medical Practice Act of 1987 Section 9 (B) (4) and Section 1285.110 b) 3) of the Rules of the Illinois Department of Professional Regulation.   • USMLE tests scores must be competitive. They should be in the 80's and passing on the first attempt is a plus. Ideally Step 2 scores should be higher than Step 1. Both tests and scores MUST be taken and reported for review. The CSA must be taken and passed.
  • Your ECFMG certification must be current and valid.
  • We do not sponsor visas of any type. You must be a US citizen or current green card/permanent visa holder (nothing pending) OR currently licensed to practice medicine in the state of Illinois.
  • All of our applications for 2009 MUST go through ERAS. No paper applications or email applications will be accepted. Our deadline for completed applications is December 1, 2008.
  • All applications must include the following:
    • Dean's Letter (or equivalent)
    • Transcripts (translated where applicable)
    • Three current reference letters
    • USMLE/Comlex Step 1 and Step 2 Results
    • Personal Statement

Interviews

Upon acceptance of your application, Lisa Stefinsky, Residency Coordinator, will contact you to arrange an interview. You will meet with the Program Director, faculty and residents. Residents will show you our facilities, take you to lunch and address your questions. Deadline for ERAS applications is December 1, 2008. Interviews will be conducted October 21, 2008 through January 31, 2009 on Monday and Friday. For out of town applicants, one night’s lodging is provided.
