Preoperative in-home Physical Therapy evaluation
for joint replacements include:

• -Preoperative evaluation and strengthening exercises when appropriate
• -Education in postoperative course of rehabilitation and outcome expectations
• -Home safety check
• -Equipment recommendations
• -Resource book on joint replacement    

Postoperative Program Includes:

• -Our home care staff works closely with physician, social services, patient and family in the  hospital, to develop a customized discharge plan, providing a smooth transition of care
• -Visit frequency based on patient needs, daily if necessary
• -Multi-discipline approach                                                   

Billed under Medicare Part B and secondary insurance, or private insurance
