Help to Keep Your Family Well: Pre-register for Your Hospital Appointment, Test or Admission
As a convenience for our patients, we are pleased to offer Online Patient Pre-Registration. Pre-registering for an upcoming appointment allows you to reduce your wait time on the day of service and ensures important information about you is accurate and up-to-date.
Before you begin
- If you feel this is a medical emergency, please call 911 or go to nearest Emergency Room.
- Please complete the Online Pre-Registration at least 2 business days before your service date. Do not use if your service date is today or tomorrow.
- You are entering a web site that is secure. The information you view and send cannot be read while in transit.
- If your appointment is scheduled and less than two business days away, please call the number below.
Information that you will need to provide:
- Your current Insurance card(s)
- Your Medicare or Medicaid card (if applicable)
- Your Primary physician's first and last name
- Your Referring physician's first and last name
- Your Social Security Number
- Your employment information
- Emergency contact information
