Eligibility for Home Health Services

  • Homebound/not driving
  • Physician’s order
  • Billed under Medicare Part A/covered 100%; or Private Insurance/per individual policy guidelines
  • Skilled need
  • Skilled Nursing
  • Medication changes
  • NG or G-tube feeding instruction
  • IV infusions/antibiotics, hydration, TPN
  • Unstable blood sugars
  • Trach care instruction 
  • Bowel/bladder training
  • Diabetic teaching instruction                                                  
  • Nebulizer treatment                                                                        
  • Ostomy care and instruction
  • Unstable blood pressure
  • Postoperative assessment of healing process                 
  • Postoperative staple removal
  • CHF monitoring
  • Abnormal/fluctuating lab values
  • Wound care instruction, assessment, plan
  • Mental status changes
  • Urinary catheter insertion/maintenance
  • Pain management, assessment & instruction
  • Infection
  • Any reason physician would request an assessment or observation

Physical Therapy

 Medical Social Work

  • Long- range and short-range planning                     
  • Advocate on patient behalf to access services                 
  • Linkage to in-home assistance and
  • Community resource information                              
  • Help develop Advanced Directives plan
  • Financial programs

Speech Therapy

  • Assessment; speech/voice production      
  • Develop control of vocal/ respiratory system for voice production                                                           
  • Swallowing disorders
  • Language communication 

Home Health Aide

For bathing and personal needs (RN or PT needed to provide this service)
